This article will show you how to add or remove a clinical location in U-CV so it no longer shows on the Find A Doc (FAD) profile. Clinical locations show under the “Clinical Information” tab on the FAD profile.

Adding a new clinical location

1. Log in to the U-CV System.

2. Click on Personal from the center navigation menu and click on the Addresses category.

3. Click Add a New Entry.

4. Click on the Address Type drop-down and select Clinical Address from the list.

5. Click in the Clinical Location field to search for a University of Utah hospital, clinic, or contracted location. Click on the location you would look to add. This will populate most of the remaining fields.

   1. If you’re unable to find the correct location or this is a new University of Utah location, enter the name and select Request this as a new value. Note: please make sure to try different search options before requesting a new value.

   2. A new field will appear where you can enter additional information about this location. Please enter as much information as you can, including street address, room number, city, state, zip code, and phone number.

6. If you’d like, enter a phone number and/or fax number. This should be the number patients should call to reach the clinic or schedule an appointment.

7. If this is the first address you’re entering or if this is the address you want to show on the CV format, check the Primary Address box. If you do not want the address to show on your online profile, uncheck the Display on U of U Profiles box. Click Save.

To remove a clinical location

1. Log in to the U-CV System.

2. Click on Personal from the center navigation menu and click on the Addresses category.

3. Click on the Delete icon next to the address you want to remove.

4. Click Yes on the Delete Activity? prompt.
