Addresses

Includes academic and clinical office, and home addresses. One address needs to be specified as primary for mail/contact, which flows into the Find a Researcher and Faculty Finder applications. Clinical and Office addresses can be displayed on Find A Doctor. An approved list of buildings is provided to assist in address input.

Administrative Duties

Highlighted experience that could also be listed in Career or Service. Duplication is allowed, but the entries should not be identical. The Administrative Duties entry should describe administrative aspects of activity and typically apply to leadership and other administrative roles specifically.

Example: Chief Resident, University of Utah School of Medicine Radiology, organized resident rotation schedule, restructured resident call assignments, lead monthly resident meetings.

Internal Teaching Experience

Presentation of a clinical or research topic to internal trainees and other faculty for discussion and education purposes that is not part of a formal course. Includes Clinical and Research Conferences: Case Conferences, Morning Report, Noon Conference, Research in Progress, Journal Club.

Example: Telemedicine and Medical Oncolog, Noon Conference, Arizona Cancer Center, 15 fellows.

Various types of phone numbers important for reporting and contacting. Types of phone numbers managed here are: Primary, Mobile and Pager.

  • Primary - Derives from University of Utah Human Resources and may not be changed here. If primary phone number is incorrect, contact your payroll reporter.

  • Mobile - Mobile phone number which may be used for reporting and/or contacting, if applicable. This phone number may be used by the "University of Utah Health - Referring Provider Directory" if desired

  • Pager - Number for a personal or professional pager which may be used for reporting and/or contacting if applicable.

Note: Phone numbers for academic offices, labs, and clinical locations can be updated under the "Addresses" section of the UCV system.

Presentations

Address, speech or other oral presentation given at a formal meeting, conference or symposium. As appropriate, include the names of co-authors and collaborators and other descriptive information. Each presentation should be listed only once.

Note: Not all presentation types show on the SOM CV format. Only the following: Invited/Visiting Professor Presentation, Keynote/Plenary Lecture, Meeting Presentation, Outreach Presentation, Industrial Presentation, Peer-Reviewed Presentation (shown under Meeting Presentations), and Poster.
