Request eSurvey questionnaire for reappointment voting
University Faculty Information & Support (UFIS) assists departments in creating their annual reappointment voting ballots in the MBM eSurvey system. Departments can submit a template that includes faculty members' names and uNIDs and that template is used to create the questionnaire in eSurvey with “Reappoint” selected as the default response.
Please allow one week for us to create the questionnaire and three days to pull the results.
Instructions
From the UFIS Help Center, click Need to raise a request? Contact us > Submit a request or incident.
Fill out the required fields and attach the completed standardized excel template.
Click Send.
A UFIS team member will notify you when the questionnaire is ready. Once the vote is finished, please deactivate the campaign in eSurvey and submit another request to pull the results.
If you would like a separate questionnaire created for adjunct faculty, you do not need to create a separate request, but you must attach a separate spreadsheet.
